Physical Evaluation Forms

All student athletes and marching band members are required to have a yearly physical before participating in any SFS-sponsored Athletics programs. Please click here to access the "Pre-Participation Physical Evaluation Form and Parental Consent". 

This form must printed and brought to your student's physical, signed by a qualified physician, physician’s assistant, or an advanced practice nurse covered by professional liability insurance, and returned to your student's coach prior to participation.
